Scoresheet

Please use the USAW Official Scoresheet in one of the formats below to submit results to the National Office. Download the form to your computer, fill it out completely (all columns, including membership numbers), and then email the results to usaw@usaweightlifting.org.

PLEASE NOTE:  All participants of USA Weightlifting sanctioned events must be current USA Weightlifting members BEFORE they will be allowed to lift. Membership forms completed at the meet, membership payments, and meet results must be received by the National Office no later than two (2) weeks after the meet.

Failure to comply with these requirements will cause the entire meet results to become null and void.
